6.412 - Physical Examinations and Immunizations

Robertson County School Board Policy


PHYSICAL EXAMINATIONS
The principal shall ensure that there is a complete physical examination of each student prior to:
  1. Entering Tennessee Public School for the first time. This applies to preschool, pre-k, kindergarten, first grade and other students for whom there is no health record (TRR/MS 0520-1-3-.08(2)(a).; TRR/MS 1200-14-1-.29; TCA 49-6-5002, 49-6-5003, 68-1-103, 68-5-103, 68-5-105(a);
  2. Transferring students from in-state public schools will have 30 days to show proof of physical examination; and
  3. Participation as a member of any athletic team or in any other strenuous physical activity program.
  4. Cost of the examination shall be borne by the parent or guardian of the student. These records shall be on file in the principal's office. (TRR/MS 0520-1-3-.08(2)(b)
IMMUNIZATIONS
No students entering school, including those entering preschool, pre-k, kindergarten or first grade, those from out-of-state nor those from nonpublic schools, will be permitted to enroll (or attend) without proof of immunization, as determined by the Commissioner of Public Health. (TCA 49-6-5001(a); TRR/MS 0520-1-3-.08(2)(a)  It is the responsibility of the parents or guardians to have their children immunized and to provide such proof to the principal of the school which the student is to attend.(TRR/MS 0520-1-3-.08(2)(b)

Exceptions, in the absence of an epidemic or immediate threat thereof, will be granted to any child whose parent or guardian shall file with school authorities a signed, written statement that such measures conflict with his/her religious tenets and practices; or due to medical reasons if such child has a written statement from his/her doctor excusing him from such immunization.(TCA 49-6-5001 (b)

Proof of exceptions will be in writing and filed in the same manner as other immunization records.

A list of transfer students shall be kept at each school throughout the school year in order that their records can be monitored by the Department of Health and Environment.
